Hello Ryan,
Thank you for putting together WP-ContactForm and making it public. It’s one of the best plugins I’ve used for WP and I really like the seamless way it fits in.
Hi, great plugin to start out. I am now having an odd problem though. I have the 1.1 version installed, and for the longest time it worked. The contact form showed up on the page I had it on. Now all of the sudden, it does not show up at all. I haven’t made any changes to it or anything either.
The page url I have it set on is: http://www.mindwiped.com/gurlrider/?page_id=38 …you can see that it isn’t showing up properly now. Any ideas as to what’s wrong? I even tried making a new page with a new contact form and that didn’t work as well. Kinda lost here.
Might be worth mentioning that because of the use of get_permalink() on line 137 of wp-contactform.php, this plugin requires permalinks to work. Before I got permalinks working I would get a 404 page after clicking “Send”.

If you’re using IIS like me, you might need to follow the instructions at http://codex.wordpress.org/User:ringmaster/IIS to get permalinks working.
Cheers.
